1. Agency Number
This is the number that identifies the agency responding to an incident.
2. Jurisdiction Incident Number
This is the unique number for each incident reported to dispatch.
3. Incident Location City/County-FIPS
The code for the city, town, or county where the patient was found.

4. Location Type
Type of location the incident occurred in.
It is classified as: Home or residence, farm, mine or quarry, industrial place,
place for recreation or sport, street or highway, public building, residential
institution, educational institution, other specified or unspecified location,
or unknown.
5. Type of Service
This is the type of service requested.
It is classified as, Scene, Unscheduled interfacility transfer, Scheduled
interfacility transfer, standby, rendezvous, or unknown.
6. Incident Disposition
The end result of EMS response.
This is classified as, Treated and transported by EMS, Treated and transferred
care, Treated and transported by private vehicle, Treated and released, No
treatment required, patient refused care, DOA, Cancelled, No patient found, or
unknown.
7. Attendant in Charge
This is the level of certification of the Attendant in Charge of a call.
Classified as, FR-First Responder, EMT-Emergency Medical Technician, ST-Shock
Trauma, CT-Cardiac Technician, I-Intermediate, P-Paramedic, RN-Nurse, MD-Doctor,
OTH-Other.
8. Attendant 1
This is the level of certification of the Attendant on a call.
Classified as, FR-First Responder, EMT-Emergency Medical Technician, ST-Shock
Trauma, CT-Cardiac Technician, I-Intermediate, P-Paramedic, RN-Nurse, MD-Doctor,
OTH-Other.
9. Attendant 2
This is the level of certification of the Attendant on a call.
Classified as, FR-First Responder, EMT-Emergency Medical Technician, ST-Shock
Trauma, CT-Cardiac Technician, I-Intermediate, P-Paramedic, RN-Nurse, MD-Doctor,
OTH-Other.
10. Driver
This is the level of certification of the Driver on a call.
Classified as, FR-First Responder, EMT-Emergency Medical Technician, ST-Shock
Trauma, CT-Cardiac Technician, I-Intermediate, P-Paramedic, RN-Nurse, MD-Doctor,
OTH-Other.
11. Date Incident Reported
Date the call is first received by a Public Safety Answering Point (PSAP)
12. Time of Call
Time the call is first received by PSAP.
13. Dispatched Time
Time the response unit is notified by EMS dispatch.
14. Responding Time
Time the response unit begins to respond from the building.
15. Arrive Scene Time
Time the response unit arrives at the scene of the call.
16. Arrive at Patient
Time the response unit makes contact with the patient to begin assessment.
17. Leave Scene
Time the response unit leaves the scene and heads toward the hospital or other
location.
18. Arrive at Destination
Time the response unit arrives at the destination or transfer point.
19. Leave Destination
Time the response unit leaves the destination or transfer point.
20. Return to Service
Time the response unit arrives back at quarters and is available for a response.
21. Patient's City/County FIPS of Residence.
Patient's residence code.

29. Mechanism of Injury
External cause of injury.
Classified as: Aircraft related accident, Assault, Bike accidents, Bites,
Burns-Thermal/Chemical, Chemical poisoning, Drowning, Drug poisoning,
Electrocution (Non-lightning), Excessive cold or heat, Falls, Firearms injury,
Lightning, Machinery accidents, Mechanical suffocation, Motor vehicle accidents
(public/non-public roads), Pedestrian traffic accident, Radiation exposure,
Smoke inhalation, Sports injury, Stabbing, venomous stings (plants or animals),
water transport accidents, Other, or Unknown.
30. Injury Description
Clinical description of injury type and body site.
Body Site Classification:
Head only (excluding neck, cervical spine, and ear), Face (including ear),
Thorax (excluding thoracic spine), Abdomen (excluding thoracic spine), Spine,
Hand and/or arm, Foot, Leg, or bony pelvis, Unspecified body region.
Injury Type Classification:
Swelling/bruising, Blunt Injury, Laceration, Deformity, Puncture/Stab, Gunshot,
Amputation, Crush, or Burn.
31. Signs and Symptoms
Signs and symptoms reported or observed by the provider.
32. Provider Impression
Provider's clinical impression which led to the management given to the patient
(treatments, medications, or procedures).
33. Blood Pressure
Patient's systolic/diastolic blood pressure
34. Pulse Rate
Patient's palpated or auscultated pulse rate expressed in number per minute.
35. Respiratory Rate
Patient's unassisted respiratory rate expressed as a number per minute.
36. Respiratory Effort
Patient's respiratory effort
Classified as: Normal, Increased-Not Labored, Increased-Labored OR
Decreased-Fatigued, or Absent.
37. Skin Perfusion
Patient's skin perfusion, expressed as normal or decreased.
Sets 38, 38, and 40 pertain to the Glasgow Coma Scale (GCS) and how it relates to the patient.

39. GCS for Verbal - Values 1 - 5

41. Time of Witnessed Cardiac Arrest
Time of a witnessed cardiac arrest
42. Time of First CPR
Time or best estimate CPR was commenced on the patient.
43. Provider of First CPR
Person who provided the first CPR on the patient, classified either as a
bystander or a responder.
44. Time of First Defibrillatory Shock
Time of the first defibrillatory shock was administered

45. Provider of First Defibrillation
Person who provided the first defibrillation. Classified as bystander or
responder.
46. Defibrillation Device
Type of defibrillator used on the patient. Classified as Automatic
External Defibrillator (AED), or Manual.
47. Time CPR Discontinued
Time at which medical control or responding EMS unit terminates resuscitation
efforts (chest compressions and CPR in the field).
48. Time Circulation Returned
Time of restored palpable pulse following resuscitation efforts in the field.
49. EKG Initial Rhythm
Initial monitored cardiac rhythm as interpreted by EMS personnel.
Classified as: Normal Sinus Rhythm, Other rhythm from 60-100, Paced
Rhythm, Bradycardia, Extrasystoles, Narrow Complex Tachycardia, Wide Complex
Tachycardia, Ventricular Fibrillation, Asystole, Pulseless Electrical Activity
(PEA), or Other.
50. EKG Last Rhythm
Monitored cardiac rhythm upon arrival at the destination.
Classified as: Normal Sinus Rhythm, Other rhythm from 60-100, Paced
Rhythm, Bradycardia, Extrasystoles, Narrow Complex Tachycardia, Wide Complex
Tachycardia, Ventricular Fibrillation, Asystole, Pulseless Electrical Activity
(PEA), or Other.
51. Procedures or Treatment
Identification of procedures attempted or performed on the patient.
Classifications are:
Assisted ventilation, Chest decompression, Cricothyrotomy, EGTA/EOA/PTL, ET,
Nasal airway, NG Tube, Oral airway, Oxygen-cannula, Oxygen-Mask, Backboard,
Bleeding controlled, Burn care, CPR, EKG monitoring, Defibrillation,
Immobilization of Extremity, Spine, or Traction Splint, IV, Intraosseous

52. Treatment Authorization
Indicates the type, if any, of treatment authorization.
Classified as: Standing Orders, On-Line (radio/telephone), On-Scene,
Transfer Orders, DNR, or Unknown.
53. Motor Vehicle Impact
Motor vehicle impact site during the collision.
Classified as: Head-on, Lateral, Ejection, Rear, Rollover, Rotation, or
Unknown.
54. Safety Equipment
Safety equipment in use by the patient at time of injury.
Classified as: None used, Shoulder belt only, Lap belt only, Shoulder and lap
belt, Child safety seat, Airbag deployed, Helmet, Eye protection, Protective
clothing/gear, Personal flotation device, or Unknown.
55. Level of Care Provided
Type of care rendered by personnel regardless of level. Classified as
Advanced (ALS) or Basic (BLS) Life Support.
56. Destination Transferred
Heath care facility or prehospital agency that received patient from EMS
provider.
57. Destination Determination
Reason a transport destination was selected.
58. Receiving Facility
Specific health care facility or prehospital agency that received the patient
from EMS provider.
